To 200 tests/day capacity. Which, to my ears, means we're woefully underprepared for this. That isn't nearly enough. I've heard the UK is starting to test randomly to determine the spread. I think it's gonna be out there. Dealing with the sick people and trying not to get sick seems more important than mass testing now that it does seem uncontainable

That guy is doing amazing work. He was already studying viral infection spread in the Seattle area using genetic analysis. Basically the perfect forensic researcher to have on hand. Amazing how they can trace the genetics from case to case like that.

But yea, 6 weeks of time to spread. The man mentioned Travis Bedford estimates a few hundred infected already. Remember people can be asymptomatic and still spreading

Now that we know 6 weeks of a spread to in all likelihood a "few hundred" if not more, makes you wonder. My boss's family and my coworker were all down for the count recently. Who knows. Northshore SD is a group of schools in the suburbs JUST north of Seattle.

my kid goes to a school in the district and we just got an email from the district superintendent that kids can go to school on Monday but if you hold them out it will be an excused absence, and that Tuesday schools will be closed to prep staff on how to provide instruction to students if the schools have to be closed long-term.

this comes after a staff member at a school in the district (not my kid's school) has flu-like symptoms and is expected to test positive. on top of that there's a nursing/retirement center type place in Kirkland (yes, where the Costco brand's name comes from) where the the coronavirus seems to have spread that families have been visiting, including kids and you expect some of those families to have children in the district or for kids in the district to have visited that location.

it seems like this situation is going to get worse, so i don't see why the schools won't close when they are doing this already.

they (Chinese government) basically hid this shit for the first month it was known of. that gave people enough time to travel about asymptomatic spreading it all willy nilly and getting mis-diagnosed and shit.

i mean, they are saying it has been in washington state for well over a month now.

the bad thing as far as spreading globally is this shit popped up on the cusp of chinese new year when millions of chinese travel to and from china.

i hope this gets reel'd in asap, and i'm very curious to see how this affects China politically in the aftermath. there are 10s of millions of Chinese people who are super pissed off for being lied to about this and then the subsequent quarantine and how citizens are being treated.

i'm sure this is wishful thinking on my part, but it would be wild if it was THIS that led to a democratic (or more democratic) China.

i've been closely following who has gotten this and how they've fared.

basically everyone who has died is old or compromised in some other way.

some confirmed cases show no symptoms (which is a bit scary on another level since you're probably spreading it and neither you nor anyone else knows) and most others had a mild flu like stage that passed without being noteworthy.

and you're starting to see a push back against the news outlets telling them to chill the fuck out.

Look at the number of hospital beds per person in Japan, vs. what we have in the US: https://en.wikipedia.org/wiki/List_of_OECD_countries_by_hospital_beds

And the country is actually doing something. The US .gov has basically set us up for a big surprise (for many people) by saying little and doing nothing. The .gov chose the economy & keeping panic low as the better option a long time ago.

I don't think things will go as smoothly here in the US.

Not massive panic but EVERYONE was buying 2-4 cases of water... i know it's costco lol but more than normal, and I've never seen the top racks that bare.

But it's a snowball thing. I wound up buying extra water, tp, and batteries not so much from being worried about the virus but in case there winds up being shortage of these things.

Med professionals are already telling people to stop buying up all the masks. Not only are they not gonna keep you from getting the virus but people are cutting in to hospital supplies

Reminds me of after the 94 quake in LA. I worked at a grocery store. Every time an aftershock hit people poured in to stock up on water

and i agree it's probably overblown, but these things take on a life of their own.

i wound up buying extra water in case people buy up all the water. it's something I would buy anyway. I just bought more. I'm in Costco pretty much every week, and i've never seen empty upper racks like today. There are usually 5 different types of water. Today, just one.

plus it's good to be prepped anyway. not just for this but anything. earthquake, etc..

as I said in my post about the school dist. above, there was a breakout at a nursing care facility. this is why there's a huge jump in reported cases and deaths here. almost ALL of those people (who aren't staff) have compromising conditions.

younger, healthier people appear to be getting mild, flu-like symptoms.

some people are testing positive with NO symptoms.

people are freaking out for, i won't say NO reason, but unnecessarily for sure.

Being prepared isn't panicking, and by being prepared you can avoid panicking. Look at what happens in hurricane zones EVERY TIME a few days before the storm. Being one of those panicked buyers is what you want to avoid.

reading your reply from the other post made me take a look at my situation. Not even on some end of the world scenario, but say like a week without power. I make enough loot and have tons of storage in the crib where. I could have a little setup with bottle waters, Gatorade, non perishable items, maybe some shit stored in vacuum containers, a little propane type stove. So, good looking, for that. I really need to go ahead and bust out for a nice generator, for real.

I read that older folks and those with other illnesses like diabetes and high blood pressure are at higher risk. The symptoms are similar to a bad cold or the flu.

the "confirmed cases". Ive had a cold for the past week after getting back from traveling to SF. It could be coronavirus or could just be a cold, who knows? They are still saying "If you didn't go to China or contact someone who did then don't bother us". I think we know by now that that's not good enough.

theres been something going around the past month! My parents have had it and been on 2 difference antibiotics, and are still sick. Not saying its corona, (it probably isnt) but this is some type of non normal flu going around in our area.

TAMPA, Fla. - Friends who traveled took a vacation to Italy have tested positive for the COVID-19 coronavirus in Hillsborough County, and now one of them insists she tried to warn authorities that she was sick before she boarded her connecting flight from New York to Tampa International Airport.

The two women – who represent two of the three coronavirus patients in Florida – are both their 20s. They recently got back from a trip to Italy, a country under a travel warning by the CDC.

The woman says they had masks with them and wore them as a precaution in Italy, and also used sanitizing wipes. But during the last few days of their trip, one of them began feeling unwell. They both got a plane back to the U.S., and the already sick woman noticed her health getting worse.

Once she landed in New York, she says she had a fever. The 29-year-old said she called the CDC to let them know where she had been and how she was feeling but, despite her flu-like symptoms and travel history, she says she was cleared to fly from New York back to Tampa.

"No one sounded prepared. No one at the CDC sounded prepared or educated on what was going on. When talking about an outbreak in Milan, Italy, I think they should know what part of the country that’s in," she told FOX 13 by phone.

It wasn't until she landed in Tampa that she was tested for novel coronavirus COVID-19.

While she wants to remain anonymous, she said she's frustrated she wasn't taken seriously, sooner.

“I 1,000% had symptoms when I landed in the states. I don’t blame Tampa. We were coming in from another domestic flight,” she added.

That was a week ago. Since then, she and her friend have been in self-quarantine at her house, remaining out of contact with anyone. The women say they are both doing OK, just trying to recover.

She told FOX 13 News they had groceries delivered to her home.

"It feels like we’re the test dummy. We are walking everyone through the procedure for the next bout of confirmations.”

They declined to say specifically what flights they were on. They did say they wore protestive masks during their travels and they have since provided their travel details to the CDC.

FOX 13 has reached out to the CDC and we are awaiting an official response to these claims.

Meanwhile, Tampa Airport spokesperson Emily Nipps told FOX 13 that the airport has "stepped up" its cleaning and sanitation efforts, "particularly around high-touch areas." Nipps said extra hand sanitizer and signage were being placed throughout the facilities.

coronavirus is NOT the flu

from many medical professional's estimates, it could be way worse. some are saying its more like a cross between SARS and AIDS with the way it rapidly attacks both the lungs and the immune system.

"could" is an important word because there is still so much unknown about the virus. the SAFEST thing to do is to treat it like its way worse than the flu. its better to lose money than thousands of lives.

in addition, early reports from recovered patients and autopsies are worrying as it's possible that even those that recover could have irreversible lung damage. Again, that isn't confirmed yet, but its concerning enough to take stopping the spread of something we don't know everything about extremely seriously.

All major gatherings in the US of over 1000 people should be cancelled in the U.S. asap.

recipe for disaster.

1 - this ain't china. in a non-authoritarian country, short of martial law you can't do what they did as far as closing down entire cities with millions of people. y'all really think they gonna build 3 new hospitals in 10 days in the US? they gonna give the contracts to some distant trump crony who never even built a fuckin 7-11 before.

2 - unlike in other countries where healthcare is free or heavily subsidized, people here will be worried about paying for getting checked out, for treatment. and, left to our own devices, ppl here go to work sick b/c they don't get sick days. definitely not for the low wage jobs where you be having the most customer contact.

3 - the disease spread was damn near geometric in china in spite of those crazy efforts. we fucked around over here (and by 'we' i mean the bitchass trump administration) for over a month. STILL don't have tests (nc governor just said today that they have 150 tests, with another 500 coming 'soon'). meanwhile s korea is testing everything that moves. but they are ahead of us as far as the disease spread.

4 - we got mad fatalities already, but are not testing ppl they've come in contact with. and this is transmissible asymptomatically, so who knows how many people have been infected.

bonus - trump overruled the cdc and flew the cruise ship ppl home, mixing infected and uninfected individuals to quarantine at a military base in CA. then, for good measure, they flew 14 personnel from health and human services to meet with quarantined people, face to face, with no fucking protective gear. and those 14 civilians then dispersed, staying in local hotels, flying domestically, doing what the fuck ever.

i could not create a fictional account of a more fucking stupid approach to a potential pandemic.

for all those on that "flu kills more people"... from chinese data (who knows how accurate), it is around 3.4% mortality, which is approx 34x more deadly than the flu. if it is as transmissible as the flu, that's maybe 1.5m deaths in the US. s korea has been going ham with the testing, so their numbers may be more realistic. US numbers are dumb high b/c we're not testing people, so the numbers we see of known covid-19 deaths infer at least a 30x number of infected.

there may be thousands of people who've had it and were fine that would inflate that numerator.

additionally, that one nursing home in Kirkland, near me, had an outbreak in a place full of the most at risk people that went completely unchecked. that's inflating the denominator.

person (in china, and i think it held up in s korea, where it trust the data better) infected > 2 other ppl.

china locked down entire cities. s korea has tested over 200k people.

in the US we have potentially 2 -3k infected people who could be asymptomatic and infecting 6k ppl. who will then infect 12k. then 24k. then 48k. then 96k. then 192... then 384k, then 768k, then 1.5m

if the 80% who get mild cases are never identified and controlled, we can end up with over a million people infected, with 20% of cases being major, that's 200k hospitalization cases which would overwhelm our healthcare system.

china took a huge L on their economy because they understood how serious this was. it looks like it paid off and that their outbreak has peaked.

stupid ass trump is seriously musing about letting all the people die on the ship so the US' numbers don't go up. and put all business and pr type people in charge of this to try to keep from fucking up the stock market, which is already fucked up.

this is being handled in the worst possible way.

last week. we're about 2 weeks behind italy.

this thing would have been noticed if it was following this same trajectory in the wild, b/c in a local epicenter the hospitals and health care systems would have been overwhelmed. which is what is happening as it spreads and ramps here.

china says its cases have plateaued. s korea says theirs have and we have less reason to be skeptical of them. china got up over 80k cases and 3k deaths before they said they got a handle on it. using tools not at our disposal:

- real id on social media
- pervasive monitoring and facial recognition to deter ppl breaking quarantine
- ppl mad scared of the government (as opposed to, say, congressmen 'choosing' not to self-isolate, and ppl out in the midwest going to a school dance when they supposed to be on quarantine
- ability to absorb the economic shot of almost all businesses grinding to a halt
- an existing manufacturing base that could be quickly retooled to produce masks and other items needed by the effort
- ability to build 3 hospitals in 10 days.
- ability to isolate an entire region with up to 60 m ppl


china messed around in the early phases of this and tried to contain it on the low before it became public.

if our numbers follow the same trajectory, it will be very very bad. if the spread is worse here (and it may be due to the incompetence of this regime), it may be catastrophic

People who get Covid-19, the World Health Organization’s shorthand for Coronavirus Disease 2019, 1 are much less likely to die than those infected with Ebola, the Severe Acute Respiratory Syndrome of 2003 and the Middle East Respiratory Syndrome first reported in 2012. And no, this is not the same as the Ebola crisis.

It’s not the same as the 2014 Ebola crisis in part because it appears to be a much bigger deal for the U.S. and other countries outside of West Africa. As Microsoft Corp. co-founder Bill Gates, also technically an amateur epidemiologist but by this point quite a well-informed one, put it Friday in the New England Journal of Medicine: “Covid-19 has started behaving a lot like the once-in-a-century pathogen we’ve been worried about.”

How do we reconcile these differing views of Covid-19? Well, I too am an amateur as an epidemiologist, but I find that charts and (very simple) equations help me think through things. On the theory that others might find this helpful, too, let’s start with the approximate case-fatality rates for the diseases listed by Mulvaney and a few others you may have heard of.

These fatality rates can change a lot depending on time and place and access to treatment. The Covid-19 rate is obviously a moving target, so I’ve included both the 3.4% worldwide mortality rate reported this week by the World Health Organization and the 1% estimate from a study released Feb. 10 by the MRC Centre for Global Infectious Disease Analysis at Imperial College London that factored in probable unreported cases. The authors of that study also said that, given the information available at the time, they were 95% confident the correct fatality rate was somewhere between 0.5% and 4%. Gates used the 1% estimate in his article, and when I ran it by Caroline Buckee, an actual professional epidemiologist who is a professor at Harvard’s T.H. Chan School of Public Health, she termed it “reasonable.”

In a context that includes Ebola and MERS, the Covid-19 death rates are much closer to those of the flu, and it’s understandable why people find the comparison reassuring. Compare Covid-19 with just the flu, though, and it becomes clear how different they are.

The 61,099 flu-related deaths in the U.S. during the severe flu season of 2017-2018 amounted to 0.14% of the estimated 44.8 million cases of influenza-like illness. There were also an estimated flu-related 808,129 hospitalizations, for a rate of 1.8%. Assume a Covid-19 outbreak of similar size in the U.S., multiply the death and hospitalization estimates by five or 10, and you get some really scary numbers: 300,000 to 600,000 deaths, and 4 million to 8 million hospitalizations in a country that has 924,107 staffed hospital beds. Multiply by 40 and, well, forget about it. Also, death rates would go higher if the hospital system is overwhelmed, as happened in the Chinese province of Hubei where Covid-19’s spread began and seems to be happening in Iran now. That’s one reason that slowing the spread is important even if it turns out the disease can’t be stopped.

Could Covid-19 really spread as widely as the flu? If allowed to, sure. The standard metric for infectiousness is what’s called the reproduction number, or R0. It is usually pronounced “R naught,” and the zero after the R should be rendered in subscript, but it’s a simple enough concept. An R0 of one means each person with the disease can be expected to infect one more person. If the number dips below one, the disease will peter out. If it gets much above one, the disease can spread rapidly.

Here are R0s for the same list of diseases as above. These are rough approximations, in most cases the midpoints of quite-large ranges. But they do give a sense of relative infectiousness.

This helps explain why public health authorities want everybody to get vaccinated against the measles. It’s not all that deadly a disease, but once it gets going in an unvaccinated population, everybody gets it.

The numbers also seem to indicate that Covid-19 is a lot more contagious than the seasonal flu. Average R0 isn’t the whole story, though. Why all the worry about MERS, for example, which with an R0 below one shouldn’t spread at all? Well, it’s extremely deadly, its R0 can rise above one in certain environments, among them hospitals, and ... you can catch it from your camel.

Then there’s SARS, which is both deadlier than Covid-19 and has a similar R0. Why was it wiped out in about a year, while some experts warn that Covid-19 may be around forever? Because SARS usually didn’t become contagious until several days after symptoms appeared. This meant that, as four British infectious disease experts wrote in 2004, “actions taken during this period to isolate or quarantine ill patients can effectively interrupt transmission.” They proposed adding another variable to disease-transmission models: the proportion of transmission occurring prior to symptoms. For SARS, this was less than 11%, probably much less. For influenza, it is between 30% and 50%, making it far harder to control the disease’s spread.

With Covid-19, “it seems that it can transmit quite a bit before symptoms occur,” Buckee says. How much is still up in the air. World Health Organization Director-General Tedros Adhanom Ghebreyesus has been arguing this week that pre-symptomatic transmission appears to be low enough that Covid-19 can be controlled in ways that the flu cannot. “If this was an influenza epidemic, we would have expected to see widespread community transmission across the globe by now,” he said Monday, “and efforts to slow it down or contain it would not be feasible.”

To understand how the spread of such a disease can be contained, it helps to break R0 down to its constituent parts. A simple formula that I got from Buckee is:

In some cases you can shorten the infectious duration with treatment. Quarantining people once you know they’re infected effectively shortens it, too. Variables b and k, meanwhile, are clearly dependent on behavior. The probability of infection is reduced by things like frequent hand-washing, replacing handshakes with fist bumps and such. The contact rate is reduced by staying home. By putting much of the country on lockdown, Chinese authorities reduced the contact rate enough that Covid-19’s R0 in the country fell below one. They also incurred huge economic and social costs. Now, as China begins to go back to work, the big question is whether a less-draconian approach can keep the disease in check or whether it will just start spreading again.

That’s the big question in the U.S., Europe and pretty much everywhere else on earth too. It can’t be answered entirely by professional epidemiologists, either. Weighing whether the costs of a particular intervention are worth the benefits is at heart a political decision. So it’s actually good that politicians are moonlighting as amateur epidemiologists. Some of them may just need to study a little harder.

I live in Dubai. GCC countries have instituted their precautions - with Saudi instituting a travel ban to and from basically all neighboring countries and Italy and canceling school for the remainder of the year. Here in the UAE, schools are closed for the next four months for cleaning and disenfecting and heavy tourist areas areas being cleaned non-stop.

The issue is however, that with most of these nations being sultanates, the government control on information is heavily regulated, if not untrue. UAE is reporting 15 cases, when the number is likely much, much higher. Doctors don't see the results, as they go straight to the authorities. Makes it hard for those on the ground to know what's really going on.

Streets won't let me chill.

Is it really out of the ordinary for nursing home to have deaths at this rate?

Cause an 80 year old person dying.. who also had other underlying issues isn’t really that serious.

My point is when people who are younger and healthier start having a rapid decline I think it’s a serious problem.

According to data from China, where coronavirus originated, the elderly are at a heightened risk. The disease has about a 2% mortality rate and about 80% of cases are mild. But in people ages 70 to 79, that fatality rate shot up to 8%. In those over 80, the fatality rate was almost 15%.

Officials have warned that the fatality rate may be skewed due to the underreporting of mild cases where infected people experience few or no symptoms.

I've got a few severely immunocompromised people in my life, but none of them are that close. So the only things I have to do to keep them safe are passive - a lot of "don't" including the rather obvious "we can just not see each other for a bit". Couldn't imagine if it were my mom. We got March 20th and 22nd March Madness games. We've got Berkshire Hathaway the first week of May. Meanwhile they're moving Seattle Mariners out of Seattle for all of March, forcing the Warriors to play to empty crowds, canceling E3 and GDC...


My full-time job is managing a bar near this event center, and winter in the midwest is ROUGH for part-timers while some other full-timers barely squeak by. It's all in the name of gearing up for patio season and these spring events (I almost forgot the Olympic Swim Trials...) and now this fucking virus is threatening to undercut an entire industry.


The weirdest part is, the Dark Desus side of me hopes the service industry side of this spirals out of control and our country can have a real conversation not about minimum wage for service workers but employer health insurance, or the single payer system most of us could actually afford. Because none of us are getting tested, we're just taking Ibuprofen and crossing our fingers. And regulars know this; casuals who read that Washington Post article this weekend are being reminded of it.


I hear a lot of people talking about working from home but I can't exactly make you a Vesper in my apartment and ship it to you on Postmates.

Broadway Suspends Performances Through April 12 Due to Coronavirus

Broadway has suspended all of its performances due to the coronavirus, the Broadway League announced Thursday.

They will be suspended through at least April 12.

The announcement comes after Governor Andrew Cuomo announced on Thursday that gatherings of 500 or more people will be suspended statewide.

Cuomo said this guidance will go into effect as of 5 p.m. Friday, but he said it was going into effect for Broadway as of 5 p.m. Thursday.

Various other arts locations, including Carnegie Hall, the Met Opera and the Met Museum, announced on Thursday that they are closing as well.

Officials at the Met Opera and Carnegie Hall say all rehearsals and performances through March 31 are closed in both places. Carnegie Hall says its cancellations go into effect at midnight Thursday.

The Met Museum will temporarily close all three of its locations - The Met Fifth Avenue, The Met Breuer and the Met Cloisters - starting March 13.

The Risk of Systemic Healthcare Failure Due to COVID-19

I think most people aren’t aware of the risk of systemic healthcare failure due to #COVID19 because they simply haven’t run the numbers yet. Let’s talk math.

Let’s conservatively assume that there are 2,000 current cases in the US today, March 6th. This is about 8x the number of confirmed (lab-diagnosed) cases. We know there is substantial under-Dx due to lack of test kits; I’ll address implications later of under-/over-estimate. We can expect that we’ll continue to see a doubling of cases every 6 days (this is a typical doubling time across several epidemiological studies). Here I mean actual cases. Confirmed cases may appear to rise faster in the short term due to new test kit rollouts. We’re looking at about 1M US cases by the end of April, 2M by ~May 5, 4M by ~May 11, and so on. Exponentials are hard to grasp, but this is how they go.

As the healthcare system begins to saturate under this case load, it will become increasingly hard to detect, track, and contain new transmission chains. In absence of extreme interventions, this likely won’t slow significantly until hitting >>1% of susceptible population.

What does a case load of this size mean for healthcare system? We’ll examine just two factors — hospital beds and masks — among many, many other things that will be impacted.

The US has about 2.8 hospital beds per 1000 people. With a population of 330M, this is ~1M beds. At any given time, 65% of those beds are already occupied. That leaves about 330k beds available nationwide (perhaps a bit fewer this time of year with regular flu season, etc).

Let’s trust Italy’s numbers and assume that about 10% of cases are serious enough to require hospitalization. (Keep in mind that for many patients, hospitalization lasts for weeks — in other words, turnover will be very slow as beds fill with COVID19 patients). By this estimate, by about May 8th, all open hospital beds in the US will be filled. (This says nothing, of course, about whether these beds are suitable for isolation of patients with a highly infectious virus.) If we’re wrong by a factor of two regarding the fraction of severe cases, that only changes the timeline of bed saturation by 6 days in either direction. If 20% of cases require hospitalization, we run out of beds by ~May 2nd. If only 5% of cases require it, we can make it until ~May 14th. 2.5% gets us to May 20th. This, of course, assumes that there is no uptick in demand for beds from other (non-COVID19) causes, which seems like a dubious assumption.

As healthcare system becomes increasingly burdened, Rx shortages, etc., people w/ chronic conditions that are normally well-managed may find themselves slipping into severe states of medical distress requiring intensive care & hospitalization. But let’s ignore that for now. Alright, so that’s beds. Now masks. Feds say we have a national stockpile of 12M N95 masks and 30M surgical masks (which are not ideal, but better than nothing). There are about 18M healthcare workers in the US. Let’s assume only 6M HCW are working on any given day. (This is likely an underestimate as most people work most days of the week, but again, I’m playing conservative at every turn.) As COVID19 cases saturate virtually every state and county, which seems likely to happen any day now, it will soon be irresponsible for all HCWs to not wear a mask. These HCWs would burn through N95 stockpile in 2 days if each HCW only got ONE mask per day. One per day would be neither sanitary nor pragmatic, though this is indeed what we saw in Wuhan, with HCWs collapsing on their shift from dehydration because they were trying to avoid changing their PPE suits as they cannot be reused.

How quickly could we ramp up production of new masks? Not very fast at all. The vast majority are manufactured overseas, almost all in China. Even when manufactured here in US, the raw materials are predominantly from overseas... again, predominantly from China. Keep in mind that all countries globally will be going through the exact same crises and shortages simultaneously. We can’t force trade in our favor.

Now consider how these 2 factors – bed and mask shortages – compound each other’s severity. Full hospitals + few masks + HCWs running around between beds without proper PPE = very bad mix. HCWs are already getting infected even w/ access to full PPE. In the face of PPE limitations this severe, it’s only a matter of time. HCWs will start dropping from the workforce for weeks at a time, leading to a shortage of HCWs that then further compounds both issues above.

We could go on and on about thousands of factors – # of ventilators, or even simple things like saline drip bags. You see where this is going. Importantly, I cannot stress this enough: even if I’m wrong – even VERY wrong – about core assumptions like % of severe cases or current case #, it only changes the timeline by days or weeks. This is how exponential growth in an immunologically naïve population works.

Undeserved panic does no one any good. But neither does ill-informed complacency. It’s wrong to assuage the public by saying “only 2% will die.” People aren’t adequately grasping the national and global systemic burden wrought by this swift-moving of a disease. I’m an engineer. This is what my mind does all day: I run back-of-the-envelope calculations to try to estimate order-of-magnitude impacts. I’ve been on high alarm about this disease since ~Jan 19 after reading clinical indicators in the first papers emerging from Wuhan. Nothing in the last 6 weeks has dampened my alarm in the slightest. To the contrary, we’re seeing abject refusal of many countries to adequately respond or prepare. Of course some of these estimates will be wrong, even substantially wrong. But I have no reason to think they’ll be orders-of-magnitude wrong. Even if your personal risk of death is very, very low, don’t mock decisions like canceling events or closing workplaces as undue “panic”. These measures are the bare minimum we should be doing to try to shift the peak – to slow the rise in cases so that healthcare systems are less overwhelmed. Each day that we can delay an extra case is a big win for the HC system.

And yes, you really should prepare to buckle down for a bit. All services and supply chains will be impacted. Why risk the stress of being ill-prepared? Worst case, I’m massively wrong and you now have a huge bag of rice and black beans to burn through over the next few months and enough Robitussin to trip out.

One more thought: you’ve probably seen multiple respected epidemiologists have estimated that 20-70% of world will be infected within the next year. If you use 6-day doubling rate I mentioned above, we land at ~2-6 billion infected by sometime in July of this year. Obviously I think the doubling time will start to slow once a sizeable fraction of the population has been infected, simply because of herd immunity and a smaller susceptible population. But take the scenarios above (full beds, no PPE, etc., at just 1% of the US population infected) and stretch them out over just a couple extra months. That timeline roughly fits with consensus end-game numbers from these highly esteemed epidemiologists. Again, we’re talking about discrepancies of mere days or weeks one direction or another, but not disagreements in the overall magnitude of the challenge. This is not some hypothetical, fear-mongering, worst-case scenario. This is reality, as far as anyone can tell with the current available data.

I'm fucked up about our lack of testing. Our ratio of death to confirmed cases is beyond a bad look. Ya know what Germany did? If you get sick then they go and test, ya mamma, ya kids, auntie, friends - anyone else you might have infected and it's doing a good job of controlling things.

Social distancing is the only way to stop the coronavirus. We must start immediately.

We don’t yet know the full ramifications of the novel coronavirus. But three crucial facts have become clear in the first months of this extraordinary global event. And what they add up to is not an invocation to stay calm, as so many politicians around the globe are incessantly suggesting; it is, on the contrary, the case for changing our behavior in radical ways—right now.

The first fact is that, at least in the initial stages, documented cases of COVID-19 seem to increase in exponential fashion. On the 23rd of January, China’s Hubei province, which contains the city of Wuhan, had 444 confirmed COVID-19 cases. A week later, by the 30th of January, it had 4,903 cases. Another week later, by the 6th of February, it had 22,112.

The same story is now playing out in other countries around the world. Italy had 62 identified cases of COVID-19 on the 22nd of February. It had 888 cases by the 29th of February, and 4,636 by the 6th of March.

Because the United States has been extremely sluggish in testing patients for the coronavirus, the official tally of 604 likely represents a fraction of the real case load. But even if we take this number at face value, it suggests that we should prepare to have up to 10 times as many cases a week from today, and up to 100 times as many cases two weeks from today.

The second fact is that this disease is deadlier than the flu, to which the honestly ill-informed and the wantonly irresponsible insist on comparing it. Early guesstimates, made before data were widely available, suggested that the fatality rate for the coronavirus might wind up being around 1 percent. If that guess proves true, the coronavirus is 10 times more deadly than the flu.

But there is reason to fear the fatality rate could be much higher. According to the World Health Organization, the current case fatality rate—a common measure of what portion of confirmed patients die from a particular disease—stands at 3.4 percent. This figure could be an overstatement, because mild cases of the disease are less likely to be diagnosed. Or it could be an understatement, because many patients have already been diagnosed with the virus but have not yet recovered (and may still die).

Juliette Kayyem: The U.S. isn’t prepared for what’s about to happen

When the coronavirus first spread to South Korea, many observers pointed to the comparatively low death rates in the country to justify undue optimism. In countries with highly developed medical systems, they claimed, a smaller portion of patients would die. But while more than half of all diagnosed patients in China have now been cured, most South Korean patients are still in the throes of the disease. Of 7,478 confirmed cases, only 118 have recovered; the low death rate may yet rise.

Meanwhile, the news from Italy, another country with a highly developed medical system, has so far been shockingly bad. In the affluent region of Lombardy, for example, there have been 7,375 confirmed cases of the virus as of Sunday. Of these patients, 622 had recovered, 366 had died, and the majority were still sick. Even under the highly implausible assumption that all of the still-sick make a full recovery, this would suggest a case fatality rate of 5 percent—significantly higher, not lower, than in China.

The third fact is that so far only one measure has been effective against the coronavirus: extreme social distancing.

Before China canceled all public gatherings, asked most citizens to self-quarantine, and sealed off the most heavily affected region, the virus was spreading in exponential fashion. Once the government imposed social distancing, the number of new cases leveled off; now, at least according to official statistics, every day brings news of more existing patients who are healed than of patients who are newly infected.

A few other countries have taken energetic steps to increase social distancing before the epidemic reached devastating proportions. In Singapore, for example, the government quickly canceled public events and installed medical stations to measure the body temperature of passersby while private companies handed out free hand sanitizer. As a result, the number of cases has grown much more slowly than in nearby countries.

These three facts imply a simple conclusion. The coronavirus could spread with frightening rapidity, overburdening our health-care system and claiming lives, until we adopt serious forms of social distancing.

This suggests that anyone in a position of power or authority, instead of downplaying the dangers of the coronavirus, should ask people to stay away from public places, cancel big gatherings, and restrict most forms of nonessential travel.

Given that most forms of social distancing will be useless if sick people cannot get treated—or afford to stay away from work when they are sick—the federal government should also take some additional steps to improve public health. It should take on the costs of medical treatment for the coronavirus, grant paid sick leave to stricken workers, promise not to deport undocumented immigrants who seek medical help, and invest in a rapid expansion of ICU facilities.

The past days suggest that this administration is unlikely to do these things well or quickly (although the administration signaled on Monday that it will seek relief for hourly workers, among other measures). Hence, the responsibility for social distancing now falls on decision makers at every level of society.

Do you head a sports team? Play your games in front of an empty stadium.

Are you organizing a conference? Postpone it until the fall.

Do you run a business? Tell your employees to work from home.

Are you the principal of a school or the president of a university? Move classes online before your students get sick and infect their frail relatives.

Are you running a presidential campaign? Cancel all rallies right now.

All of these decisions have real costs. Shutting down public schools in New York City, for example, would deprive tens of thousands of kids of urgently needed school meals. But the job of institutions and authorities is to mitigate those costs insofar as humanly possible, not to use them as an excuse to put the public at risk of a deadly communicable disease.

Finally, the most important responsibility falls on each of us. It’s hard to change our own behavior while the administration and the leaders of other important institutions send the social cue that we should go on as normal. But we must change our behavior anyway. If you feel even a little sick, for the love of your neighbor and everyone’s grandpa, do not go to work.

When the influenza epidemic of 1918 infected a quarter of the U.S. population, killing tens of millions of people, seemingly small choices made the difference between life and death.

As the disease was spreading, Wilmer Krusen, Philadelphia’s health commissioner, allowed a huge parade to take place on September 28th; some 200,000 people marched. In the following days and weeks, the bodies piled up in the city’s morgues. By the end of the season, 12,000 residents had died.

In St. Louis, a public health commissioner named Max Starkloff decided to shut the city down. Ignoring the objections of influential businessmen, he closed the city’s schools, bars, cinemas, and sporting events. Thanks to his bold and unpopular actions, the per capita fatality rate in St. Louis was half that of Philadelphia. (In total roughly 1,700 people died from influenza in St Louis.)

In the coming days, thousands of people across the country will face the choice between becoming a Wilmer Krusen or a Max Starkloff.

In the moment, it will seem easier to follow Krusen’s example. For a few days, while none of your peers are taking the same steps, moving classes online or canceling campaign events will seem profoundly odd. People are going to get angry. You will be ridiculed as an extremist or an alarmist. But it is still the right thing to do.

The entire country is on lockdown cuz 3 weeks ago, folks were saying the virus warnings were overblown and still going about their social lives.

now you got entire health systems collapsing, doctors and nurses getting sick, people with other chronic illness and ailments going without treatment and dying because beds and ventilators are reserved for Covid19 patients

the only thing we should all be doing is STAYING AT HOME.

no restaurants, bars, meetings, school etc

but people like you keep yapping.

He's talking about a stimulus package for people who SHOULD NOT BE WORKING, but HAVE TO because their work requires them to be in public.


I expect to make $20 -$40 an hour most nights (yes, the range is that broad) despite my base wage being $4.50. If I'm showing up to work - paying for Uber or riding the bus to get there - and no one comes in to drink or eat, my budget is fucked. I can't just cash in PTO. If you'd have told me six months ago to save up for a crisis that could potentially wash the first three months of good work in the calendar year I'd have planned for this, but with each event cancellation/precautionary measure, all I can do is get more SCARED, not more more relieved.


I also can't afford personal insurance because the deductibles are not in my favor as a smoker and someone who has never opened a line of credit due to student loan debt. Most service workers are in my position; neither us nor our customers know if we're carrying, and that's far from ideal.


If everyone needs to stay the fuck home then the government needs to find a way to support the people who can't. Steph Curry might be able to afford to play in an empty stadium but if I spend a full week in an empty restaurant I have to start pre-empting my landlord with questions about being a day or two late on rent while I let my power or water bills slide into overdue charges.
