It outputs Dolby Atmos, just like movies do.
So if you have a surround sound system that can decode Atmos or DD5.1 (you can set it to DD 5.1 in the settings on my AppleTV box) then it comes out in surround.
I have an SACD/DVD-Audio player that does this for physical media but many of the new release on Apple Music aren't available in physical form.
As with any surround release the quality really depends on the particular remix.
On Apple Music I think the Talking Heads catalog sounds pretty good.
It's mainly a novelty for most releases, but my physical Steely Dan disc of Gaucho is AMAZING and really the best way to listen to the album. Hopefully that comes to Apple Music. Same with Depeche Mode - their physical surround releases are great but not yet streaming.
I know Tidal and Amazon also have surround mixes too.
