in a clinical setting to ensure that it has high internal validity ie the sample population is selected based on inclusion and exclusion criteria to ensure that the trial can basically address the question, is the drug efficacious this limits the external validity- i.e. how would it work on the general population which would address the question, is it effective (and applicable in a real world population- which includes patients with comorbidities, differential adherence, etc and this is where things get interesting.
there are very few clinical trials that address the issue of external validity and id wager even the trials you have been involved in (and i am assuming you are talking about the design, selection of study participants, randomization process etc its a different story if you are coming in after there are study results
