I figure they kept fighting until Hulk tires out and turns back into Banner and Banner gets bitten. Zombie Scarlet kills an overwhelmed Hulk caught up in a horde.

On top of all that we

We do know where it came from, Wasp #1, caught a virus in the quantum realm that turned her into a zombie.
