I think her surprise was more about the manner of how Ramsey acted about it. She already knew he was an asshole. The way he acted towards her in front of other people was just dick-ish. The consummation scene though, he told her in no uncertain terms that I am going to take what I want from you, I am going to torture you, and I am going to rape you.
