I imagine they have a 53 friends-of-family-of-posse-memebers on stage holding small rags and pacing while U-God does his verse at which time they all simultaneously emphasize a rhyming word into a badly EQ'd mic and shake their rag in an upward motion .

Also, IN GENERAL - in GENERAL - raps shows can be really boring unless the act steps up and does something special.
