Is it really too hard to believe that the man realizes that he fucked-up sometime ago, and is lashing out because he's really scared and doesn't know how to get himself out of this situation?

They threw him in the 'hospital' before and he came out all fucked up. Now they (his 'family') are trying to do it again. He probably sees another 'hospitalization' or worse coming for him.
