Basically, he looks at the project of creating a civilization that can at least pretend to some degree of univocity and points out that exclusion is just as important in that project as inclusion. So as European Christians in the middle ages were picking up the smashed pieces of the Roman Empire, that meant anybody who didn't quite belong like Jews and heretics and people with physical disabilities and homosexuals and prostitutes were out. And not just out, but aggressively expelled from polite society through active persecution.
