i've gotten glasses from warby parker, coastal.com, eyebuydirect and zenni. usually there is a virtual tryon option but it's still kinda hard to see/judge how they will actually look.
if you generally know the style you are looking for and roughly the size/dimensions that will fit your head and face then you can get something that looks good. warby parker will send you five frames at a time to actually try on. also, most of the sites have a free return or exchange option if you aren't happy with how they look or fit. i've definitely had to send some back and it's usually an easy process.
the cheapest i got was free glasses from coastal and the most expensive were a hundred bucks from warby parker. the ones i'm wearing now from eyebuydirect i think were around 35.

it doesn't seem to be something that my doc normally includes on the prescipt tho.
I normally get everything done at lenscrafters. I don't recall seeing measurements since I started wearing glasses, 3-4 years ago. I think its prolly cuz the sales folks do the measures when you buy the glasses. either way, I'll get them to do it even if I don't buy from them.

I get 1 nice frame and 1 cheapie frame, and I keep the cheapie as a backup. I also do the same for shades and keep the cheapie in the car. Didn't cost me more than $100 for all of that (I have a weak prescription though). If you don't have your PD, just go to any wal-mart and ask the glasses people to measure you if you don't want to do it yourself. They don't care, lol.
Another thing I've done is bought sunglasses frames with no lens on ebay and sent them to replacealens.com to fill in. You could also take them to a random spot near you if the price is right. I did that with an oakley frogskins frame that I bought and those are my current daily glasses. I have another pair of frogskin shades, so I knew what the fit was like. If you have a frame that you like (like ray-ban wayfarers for example), that might be a good way to go.
