Ok, so...

Phylicia Rashad appeared on an episode of Blossom but I'm not sure if it was as Claire Huxtable or Phylicia Rashad. .

The Fresh Prince also appeared on an episode of Blossom as well but it was as The Fresh Prince and not the Fresh Prince of Bel Air.

Did all three shows exist in the same world?

Blossom shared OUR world and not The Cosby Show or The Fresh prince of Bel Air.

Phylicia Rashad appear as her "dream mother" on the second episode. I'd suggest this means that Blossom is in our universe and watches The Cosby Show and that's why she thinks of Claire huxtable as the dfream Moms.

blossom lived in LA - why would she picture some non-famous lady who lives in Brooklyn as her ideal mom?

This one also explained by living in our world. The Fresh Prince is a TV show that Blossom and Six watch - that's why they were singing the theme song. She jsut calls Will Smith "The Fresh prince' when she meets him because she's a dumb kid.
